Overview

As a member of an interdisciplinary healthcare team, the Hospital Pharmacist works with providers to determine optimal therapy and dosing of medications while guiding nurses and other clinical staff on administration, interactions, compatibilities, and side effects. The Pharmacist provides safe, accurate and optimal medication regimens throughout the inpatient stay while following all Federal and State regulations.

Responsibilities

Product Verification

  • Ensures medication being dispensed is accurate and in date.
  • Ensures provider orders are checked for appropriate dosing, interactions, and overall appropriateness.
  • Ensures medications are secure and not accessed by individuals who shouldn't have access.

Clinical Services

  • Provides accurate admission medication reconciliation.
  • Provides dose adjustments, route changes, and antibiotic recommendations.
  • Participates in groups and or committees relating to medications.

Development and Education

  • Educates patients and staff on medications or medication classes.
  • Provides continuing education within the department to enhance staff knowledge.
  • Participates in the pharmacy college student rotation program.
  • Ensures proper storage, preparation, transport, and disposal of medications within the hospital.
  • Delegates and provide direction to pharmacy technicians in the absence of the Director of Pharmacy.

Basic UPH Performance Criteria

  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications

Minimum Requirements

Education:

Bachelor of Science in Pharmacy or PharmD from accredited College of Pharmacy

Completion of a hospital-based or other applicable residency program

Experience:

1 year of prior hospital experience

License(s)/Certification(s):

Current licensure in good standing with the Iowa Board of Pharmacy

Valid driver's license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:

* Writes, reads, comprehends and speaks fluent English.

* Microsoft Office - basic skills.

* Customer/patient focused.

* Critical thinking skills using independent judgment in making decisions.

Other:

Use of usual and customary equipment used to perform essential functions of the position.
